reservation fee which will be subtracted from your total cost of $840
reservation fee is non-refundable
Payment plans are available as follows:
Option 1- Total cost of program ($840)- payments of $280.00 on the first day of
class then $280.00 on the 2nd & 3rd Thursdays of class.
(this option will not save your seat, it is 1st come 1st serve).
Option 2- If you have already paid the $200 reservation fee ($840-$200= $640)- payments of
$213.34 on the first day of class then $213.33 on the 2nd & 3rd Thursdays of class.
(by paying the Reservation fee it guarantees you a seat in the class, based on availability)
Ohio student disclosure course registration fee of $25 as required by rule 3332-1-22.1 of the
Administrative Code. To be paid by the student at the time of taking the course online, prior to the 1st
day of class. To complete the Disclosure course, go to: https://oh-student-course-info.edvera.com/
and click on Students- enroll in course. Then follow the instructions.
when you receive your certificate by email, please forward the email to
email@example.com so I can print it off and put it in your file.
This is a one time fee payable only once for all classes, not per class and is good for one year
The Phlebotomy Technician Program is designed to provide students with the knowledge & hands-on
skills to perform competently as a skilled Phlebotomist. Students will receive a solid background in
the fundamentals of anatomy & physiology of venipuncture, professional patient relations, accurate
record keeping, standard precautions & safety regulations (OSHA). Upon successful completion,
students will be eligible to take the optional, National Phlebotomy Certification Exam they will gain
the credentials of a Nationally Certified Phlebotomy Technician (CPT). Must Have High School Diploma
or GED, must be a minimum of 18yrs old. Must show proof of age and copy of high school diploma or
GED at time of registration. Classes must have a minimum of 2 per class or it may be cancelled. If
classes are cancelled due to low enrollment by the school, all fees will be refunded for that class only.
